Heritage DNA Services (“Company,” “we,” “us,” or “our”) provides mobile specimen collection services, mobile phlebotomy services, DNA testing coordination services, and laboratory courier coordination in California.

By scheduling, purchasing, or receiving services, you agree to the following Terms of Service.


1. Nature of Services

Heritage DNA Services provides mobile specimen collection only. We do not provide medical diagnosis, medical treatment, or laboratory testing services.

Laboratory analysis is performed by independent third-party accredited laboratories.

Our service fee covers mobile collection and specimen handling only unless otherwise stated in writing.


2. Authorization to Review Medical Documentation

By uploading or submitting laboratory orders, DNA documentation, or related materials, you authorize Heritage DNA Services to review such documentation solely for the purpose of coordinating specimen collection and laboratory delivery.

All information is handled in accordance with applicable federal and California privacy laws.


3. Scheduling & Appointment Windows

Appointments are scheduled within a designated arrival window unless otherwise confirmed.

Standard appointments include a two (2) hour service window (example: 8:00 AM – 10:00 AM).

Exact-time appointments may be offered under premium service levels and must be confirmed prior to booking.

After payment is received, the patient or responsible party will be contacted to confirm scheduling details.


4. Cancellations, Rescheduling & Refund Policy

Cancellations or reschedule requests must be made at least one (1) hour prior to the confirmed service window.

No refunds will be issued for:

• Failure to be present during the confirmed service window
• Service denial at the time of appointment
• Cancellations made less than one (1) hour before the appointment window
• Incomplete or invalid laboratory documentation

If a refund is approved, administrative or processing fees may apply.

Refunds will be processed in accordance with California consumer protection regulations.

5. Service Limitations & Scope of Practice

Heritage DNA Services technicians:

• Do not collect specimens from indwelling catheters or colostomy bags
• Do not perform wound cultures
• Do not provide wound care
• Do not administer medication
• Do not interpret laboratory results

Therapeutic phlebotomy requires a valid physician order.

We reserve the right to decline or discontinue service if collection conditions are unsafe, unsanitary, or outside the lawful scope of mobile phlebotomy practice under California regulations.


6. Laboratory Fees & Insurance

Heritage DNA Services is not responsible for:

• Laboratory processing fees
• Charges from third-party laboratories
• Insurance claim denials
• Non-covered laboratory charges
• Laboratory processing delays

Laboratory testing fees are separate from the mobile service fee.

Patients are responsible for confirming insurance coverage directly with their insurance provider and laboratory.


7. DNA Testing Terms

Legal DNA testing requires valid identification and strict chain-of-custody procedures.

Court admissibility depends on compliance with laboratory and identification requirements.

Personal knowledge DNA tests are not court-admissible.

All DNA analysis is performed by accredited third-party laboratories.


8. Results & Medical Advice Disclaimer

Heritage DNA Services does not:

• Analyze laboratory results
• Provide medical advice
• Interpret laboratory findings
• Discuss diagnostic outcomes

All result inquiries must be directed to the ordering provider or processing laboratory.


9. Limitation of Liability

To the fullest extent permitted under California law, Heritage DNA Services shall not be liable for indirect, incidental, special, consequential, or punitive damages.

Our total liability, if any, shall not exceed the amount paid for the mobile service fee.

We are not liable for:

• Laboratory errors
• Insurance disputes
• Delays beyond our control
• Patient non-compliance with test preparation instructions


10. Privacy & California Consumer Rights

California residents have rights under the California Consumer Privacy Act (CCPA/CPRA), including:

• The right to know what personal information is collected
• The right to request deletion of personal information
• The right to correct inaccurate information
• The right to limit use of sensitive personal information

We do not sell personal information.

11. Communication Consent

By providing your phone number or email address, you consent to receive service-related communications, including scheduling confirmations and appointment updates.

Message and data rates may apply depending on your carrier.

You may opt out of non-essential communications at any time.


12. Right to Refuse Service

We reserve the right to refuse or discontinue service if:

• Required documentation is not provided
• Payment is not received
• Conditions are unsafe
• Requested services fall outside legal scope


13. Governing Law

These Terms of Service shall be governed by the laws of the State of California.

Any disputes shall be resolved in Riverside County, California, unless otherwise required by law.
